Complete Sections 1, 2 (Check Great Rivers State for your type of plate), 3, and 4. To find your local license office code, visit the Department of Revenue website. For section 4, visit http://dor.mo.gov/offloc/ to find the location and 3 digit code of the office nearest you.

4. Write a check for $15.00 to Missouri Department of Revenue
5. Send your receipt with completed form 1716 to the Missouri Department of Revenue. Your local DMV office that you select on the form will contact you when your plates have arrived, and then you will go pick the plates up at your local DMV office.
